Scholarship for Florida
Residents
GUIDELINES
TO THE USCMA SCHOLARSHIP APPLICATION
HISTORY
This
Scholarship fund was established by The United States
Colombian Medical Association and the Auxiliaries Ladies.
This is a not for profit organization of physicians and dentists
in the USA that promotes scientific, educational, cultural
and charitable abilities in Colombia and the USA.
Two scholarships each one of $2000.00 per academic year
toward tuition at an accredited state school of student's
choice In the State of Florida. Scholarship
awards are paid directly to the school.
No funds are prepaid to scholarship ' recipients
ELIGIBILITY
REQUIREMENTS

Students
who had been born in Colombia or of Colombian parents. |
| • |
Must
be a senior of Miami-Dade or Broward Public Schools. |
| • |
Proof
of FINANCIAL NEED. |
| • |
Scholastic
Achievement GPA 3.00. |
| • |
Currently
ACCEPTED by a post secondary institution.
(College, university or vocational school)
* Enrolled as a FULL TIME student. |
| • |
The
student must maintain a GPA of 3.3 to received award in
succeeding years. |
SCHOLARSHIP

School TRANSCRIPT. |

Volunteer
/ Work Experience. |

250
words ESSAY (Typed & Double-spaced). |
| • |
Letter
of RECOMMENDATION from following: Teacher and Guidance
Counselor. |

Letter
of recommendation from a COMMUNITY LEADER (not a family
member). |
| • |
Most
recent Federal Income Tax Return of both parents. |
DEADLINE
Application
must be completed by April 16 of 2003.
